TELEHEALTH: CONVENIENT CARE, ANYWHERE
Telehealth allows a means to provide health care for a patient when the provider and the patient are not in the same location. This includes a live interaction between a provider and a patient. Visits may also include a caregiver, as appropriate.
Examples include:
- Video calls between a patient and a health care provider
- Audio-only calls when a video visit is not an option
- Secure text messaging to answer patient questions
Telehealth visits are only available for established patients.
- Please make a request for a telehealth visit while scheduling, and the staff will let you know if a telehealth visit is appropriate for your case.
- You must have a Patient Portal account to access a live telehealth with us. Once enabled, you will receive an email with your username, temporary password, and a link to the Patient Portal.
- If you do not have a portal account or have not received an email, please contact our office and request your patient portal access details.
- After scheduling your Tele Visit appointment, you will receive an email confirmation with instructions on how to begin your visit.
- On the day of your appointment, a reminder email will be sent 90 minutes prior to your appointment time, and a text reminder will be sent 10 minutes prior. You must utilize a smart device or computer with a camera and a microphone for the Tele Visit.
Our comprehensive telehealth services include:
- Chronic conditions management
- Medication management
- Behavioral health services
- Remote patient monitoring (RPM).
The transmission of patient data and clinical information to the provider can occur either through in-home devices or information entered and transmitted electronically by the patient.
